Geriatric or old age care management is the process of planning & coordinating care of the elderly to meet their long term care needs, improve their quality of life, & maintain their independence for as long as possible.
Our staff is trained in tending to the needs of people who are old and frail or people with some chronic or degenerative diseases. These elderly need continuing support once they leave hospital, as their illness may prevent them from leading an independent life.
